Paperwork can be one of the most overwhelming things about competing! Let’s break down what each form means, where to begin and some tips and tricks for what to include in your Community Service Initiative Statement and Fact Sheet!
Registering with Miss America
While technically not a form of paperwork, it is the first thing you need to do before competing - Register with the Miss America Opportunity.
By following the link and paying the registration fee, you become an official member of the Miss America Program which permits you to compete during the season. This fee must be submitted by the entry deadline of your FIRST competition. You only need to pay this fee ONCE per season.
Submitting the Miss America Agreement
The Miss America Agreement will need submitted for each local competition you compete in and is due one week prior to the competition. The contract is the document with the most information in it so do not wait until the last minute to read it. It will outline eligibility, terms and conditions of your year of service if you were to win a title, use of scholarship money, and more. You will also be asked for additional documentation, usually in the form of a Driver’s License, Birth Certificate, Passport, Voter’s Registration, or more to provide proof of age, residency, employment, and/or schooling. The full list of options for required documents can be found on page 21 of the contract. Photos or scanned documents of these items should be saved and uploaded through the contract link.
Submit your Local Application
A local application should be submitted for each local competition you compete in. Within this application you will need to provide general information about yourself in addition to listing 2-3 of your scholastic honors, accomplishments, interesting facts and information about your Community Service Initiative. This information is not shared with the judges, since they will get a separate Community Service Initiative Essay and Fact Sheet you submit.

About the Fact Sheet
The Facts Sheet allows you to demonstrate your interests and creativity, background, and vision in an easy-to-complete format. This is your opportunity to demonstrate to the judges why they should consider you for the position of Miss Local or Miss Local’s Teen. You may list as many or as few items under each category as you desire. Your only restriction is that it must follow the established format and it must fit on ONE page. Use your best judgment on what is most important for the judges to know about you. Use your space wisely!
Judge’s Community Service Initiative (CSI) Essay
About the CSI Essay
Miss America’s Community Service Initiative (CSI) is an integral part of the interview phase of competition, supporting one of the branded elements of Miss America as outlined by the “Four Points” of the crown. The “Four Points” are Style, Scholarship, Service and Success. CSI is specific to the SERVICE point of the crown. The CSI Essay allows you, as a participating contestant, to showcase and deploy your interests and creativity, to outline a plan or effort to engage within your local community. You have one page to communicate information, programs, and anything relating to your CSI that you want the judges to know! This form also must be signed and dated before submission.
Signing the Mandatory Dates Form
Contestants that become a Local Titleholder will be required to attend and participate in some mandatory events, including the Miss Ohio 2025 State Competition. This form lists details on events, including their date, so you can start planning your schedule around them now!
Signing the No Show Form
If a contestant has two unexcused absences from a local program in one competition year, June to March, she will be ineligible to compete within the Miss Ohio System for a full calendar year from the date of the second unexcused absence. By signing this form, you agree to these terms.
